We are not yet accepting insurance-funded client referrals. No healthcare contract, Medicaid rate, or CHW reimbursement arrangement is established.
For healthcare organizations
A clear, nonclinical coordination role.
We welcome conversations with health plans, hospitals, primary care practices, senior housing communities, and other organizations considering local resource navigation for older adults.
Follow-up coordination
Discuss how nonclinical follow-up and coordination activities could fit an organization’s workflow and defined population.
Contract-specific scope
Agree on population, responsibilities, staffing, eligibility, reporting, and payment terms before any contracted services begin.
Potential future services
CHW services, subject to authorization.
Any consideration of future community health worker (CHW) services would depend on applicable authorization and a defined agreement. No CHW service or reimbursement arrangement is currently established.

Initial scope: five Northeast Ohio counties.
The senior resource directory is planned and not yet available. Its initial scope is Cuyahoga, Lake, Geauga, Lorain, and Medina counties.
